- User Mapping and Matchmaking: Existing users in your K4 system will be matched with those in PCC, ensuring consistency and data integrity across platforms.
- Centralized Management via FusionOS: The integration is linked through the Fusion dashboard, providing a single point of control and visibility.
Requirements
- Legal & Access Setup:
- Fully executed Business Associate Agreement (BAA).
- Credentials for accessing the PCC Marketplace with permission to request integrations.
- Marketplace Actions:
- Log in to the PCC Marketplace to request the K4Community integration.
- K4Connect will approve the request, after which PCC activates it within 2–5 business days.
- Technical Configuration:
- Credentials for your Fusion dashboard from K4Connect.
- Specify values for care settings: Assisted Living, Independent Living, Memory Care, Skilled Nursing.
- Indicate which field identifies care setting (e.g., facilityId, bedId, or Payer).
- Display Preferences:
- Choose your preferred address display format for K4 (room number, address line 1/2, etc.).
- Health Data Decision:
- Determine whether to include health data from PCC in the integration.
Implementation

Initial Enablement Call:

A 30-minute meeting with the K4 Integrations team to discuss configuration details and exchange credentials.

Upon activation by PCC, the integration is connected to FusionOS, and initial census data sync begins.

User Matching:

Prior to the second appointment, known users from your K4 system are matched with PCC users. Any unmatched profiles are reviewed and manually linked by the K4 Integrations team.

K4Connect validates the integrity of census data, ensuring correct representation of resident admissions, moves, and discharges.

In the second session, K4Connect guides your team through integrating PointClickCare data with other systems such as K4Community.
